Step 1 Decide amount of contribution 
        $1,000 Founder’s Circle (annual campaign NOT related to Red and White Ball) 
           $500 Chairman’s Circle (specifically for the Red and White Ball) 
        $1,500 Both Founder’s and Chairman’s Circle 
        Other $5.00 minimum

Step 2 Decide method of payment

                     Indicate on form if Founder’s or Chairman’s Circle
